Who files for the single permit, and what must the employer do?

This is one permit for temporary residence and work: the wojewoda issues it on the application of the foreigner, not the employer. The foreigner files it themselves — since 27 April 2026 only electronically, in the MOS portal (mos.cudzoziemcy.gov.pl). A printed application sent by post is left without examination.

The employer's part is small but blocking: the foreigner enters your e-mail address in the form, a link to the employer's annex arrives there, and you fill it in online and sign it with a trusted, personal or qualified electronic signature. There is no paper signature in this procedure.

Until the annex is filled in, the foreigner's case stands still — they cannot finish filing. So two things are worth checking in advance: that the e-mail address they will enter is one you actually read, and that whoever signs for the company has a working electronic signature.

Our "Załącznik nr 1" sheet is a crib to copy from: the same fields filled with data from the system, plus the declarations you confirm in MOS. It is not proof of filing — the proof is the confirmation MOS generates after submission; save that to the file.
